- It is not necessary to complete the Card replacement application in order to have your card replaced. You just need to visit Opencard Customer Centre, where you submit your original card, ID and new 35x45 mm size photograph, preferably with white background; this is necessary only if you didn’t sign the agreement to keep your photograph for further use when applying for the first time. The corresponding forms have been in use since 1st June2010. On the reverse side write your name and surname in block letters. The application will be automatically generated. The application needs to be completed only if you are applying to have your old card (valid for 2 years) replaced by a new card (valid for 4 years)
- Customer centre staff will verify if the compulsory data needed for the card issue agrees with your valid ID. Foreigners will submit either a passport or valid residency permit.
- Provided that all the data in the application is in order, you will receive a signed copy of the application with bar code, date and place where you can collect your card.
- Provided that you have stated your mobile phone number or email address in the application, we will inform you as soon as the card is ready to be collected at a contact point.
- When collecting your card, take with you the copy of Card Issue Application and come to the contact point stated at the application. If you are exchanging your card due to changes of personal data, take the original card with you as well.
- After submitting the copy of Card issue application and ID, you will receive a new card (in case of a hybrid card you receive also PIN and PUK). Check that the data printed on the card agrees with your personal data.
- Providing all data on the card is correct, you will sign the certificate of card receipt, and other items receipt if applicable.
- If you are exchanging the card due to changes of personal data, loss, theft or depreciation of the card, you will be asked to cover CZK 100 service charge providing the card will be issued in standard 14 days period. Faulty card or cards with incorrect print will be replaced for free after you make complaint.
- If you applied at a Customer Centre to have your card replaced while you wait, you will be charged CZK 250 fee for the express issue of contactless card.
- Card replacement on the grounds of validity expiration is for free, providing you apply to have your contactless card issued by 14 days’ time. If you want to have your card issued while you wait, you will need to pay CZK 250 fee. If you want to have your card replaced with a card with a contact chip, the fee is CZK 300 Replacement of a contact chip Opencard, which was issued for free (between 1st August and 16th December 2008), is also subject to the CZK 300 fee.
- After you have paid the service charge, you will receive an envelope with important information related to card usage, including personal identification code (in case of the hybrid card also PIN and PUK).
- If you are replacing your old card, you should submit your original card. Staff will find out which applications are registered on the original card in the issuer’s system. If there is a parking voucher positive balance on your card, staff will offer you either payment in cash, or its transfer to the new card. Applications registered on the original card will be transferred to the new card by staff. If you are replacing a contact chip Opencard for a contactless Opencard, data saved on the contact chip will not be transferred as the contactless card doesn’t have the necessary storage for this data.
- Staff will destroy the old card, and return it to the operator.
