- Open, complete and print the interactive Opencard application form (PDF)(directions for completing the form). You can also print and complete a Standard application form (PDF), clearly and in block letters, or you can visit an Opencard contact point where the application form is also available.
- If the reason for the card replacement is a change in your personal data, state your new personal data on the application form.
- Attach a passport size photograph, 35x45mm, preferably with a white background. Write your first name and surname on the reverse side in block letters.
- Submit the completed application form and photograph at one of the selected Opencard contact points. Tell the assistant that you want to exchange the card. They will help you to complete the number of the card and will mark the application as a card replacement.
- On submission, provide a valid ID for verification of your personal data; foreigners should submit a valid passport or valid residence permit.
- If the application is in order, you will receive a signed copy of your application form with its barcode, the place, and date of card collection.
- If you state your mobile phone number or e-mail address, you will be notified immediately when the card is ready to be collected at the Opencard contact point.
- When you come to collect the card, bring a copy of your application form and go to the Opencard contact point that is stated in the application form. If the reason for the card replacement is a change in your personal data, also bring the original card.
- After you submit the copy of your application form and ID card, you will receive a new card (if you applied for the hybrid card, you will also receive PIN and PUK). Check whether the information printed on the card agrees with your personal details.
- If all the details are correct, submit your original card. Staff will locate applications that are registered on your card in the issuer system. If your parking voucher balance is above zero, the staff will offer either to reimburse you or to transfer it to the new card. Applications registered for the original card in the issuer’s system will be transferred to the new card by staff.
- Staff will destroy your original card. The destroyed card will be returned to the service card centre.

* Issue of the basic Opencard will be free of charge at least until 31st March 2009.
The fee can be paid in cash at an Opencard Contact Point. Upon payment you will receive a receipt of payment.
