Change of Personal Data

If you need to change your name or photograph on Opencard you have to replace the card. For more information on how to proceed, read  the Instruction for Opencard replacement.

Opencard is valid for 4 years. All the data printed on the card and held in the database must be correct and up to date. It is necessary to update any relevant changes.

Change of contact details (this is free of charge):

1. Complete the form "Notification of personal data change (PDF)" which is available at the Opencard contact point. State your new contact details on the form and sign the form.
2. If you are declaring a change of permanent address, you have to submit a valid ID card for verification of the required data; foreigners should submit a valid passport or valid residence permit.
3. Submit the completed form at the Opencard contact point.
4. If your application is in order, Opencard will change your contact details within 24 hours from the submission of the notification form.

If you are changing your name or photograph, it is necessary to apply for a replacement Opencard. You will have to hand in the old invalid card.
