- In cases of loss, we recommend to block the card to avoid any misuse. You can block the card either by using of the customer service phone line, or at a contact point. (Detailed instructions can be found in the section Blocking and Unblocking)
- Once you have blocked the card, you can apply for a new card. Open the Opencard Application Form (PDF). You can either complete the form directly on your computer (instructions) and print 2 copies, or you can print 2 blank copies of the form and complete them by hand, clearly and in block letters. You can also go directly to the Opencard contact point and collect the form from there.
- Attach your passport size photograph, 35x45 mm, preferably with white background. Write your full name on the back in capital letters.
- Hand over the completed application form and photograph at one of the selected contact points. Notify the staff that you want a new card as a replacement for the lost one. They will help you to fill out the card numbers and mark the application as card exchange.
- On submission of the Application, provide your valid ID for the data verification. Foreigners must submit either a valid passport or residence permit.
- Providing the Application is in order, you will receive a signed copy with barcode, date and place of card collection (date of issue is only for reference).
- You will be asked to pay 200 CZK handling charge.
- If you stated your mobile phone number or e-mail in your application, we will inform you immediately when the card is ready for you to be collected from a contact point.
- Take the copy of your Application form and go to the contact point stated in the Application form, or wait until the card is delivered to you by post.
- If there were some PID (Prague Integrated Transport) vouchers downloaded on your lost card, the contact point staff can download duplicates onto your new card.
