- In cases of loss, we recommend to block the card to avoid any misuse. You can block the card either by using of the customer service phone line, or at a contact point. (Detailed instructions can be found in the section Blocking and Unblocking)
- Now you can apply for a new card. You don’t need to complete an Opencard application form. Just visit Opencard customer centre, submit your ID and a new 35x45 mm size photograph, preferably with white background (necessary only if you didn’t sign the Agreement to keep your photograph for further use when applying for the first time. The corresponding forms have been in use since 1st June2010). On the reverse side write your name and surname in block letters. The application will be automatically generated. Inform the staff that you want a new card as a replacement for the lost one.
- You have to submit your valid ID when applying; foreigners are required to produce either a valid passport or residency permit. In case of loss or theft of the ID, you have to submit either passport or a replacement ID document together with any document with your name, date of birth and photograph.
- Provided that all the data in the application is in order, you will receive a signed copy of the application with bar code, date and place where you can collect your card (date of issue is just approximate)
- You will be asked to cover CZK 100 service charge providing the card will be issued in the standard 14 days period. If you want to have your card issued while you wait, you will need to pay CZK 250 fee (unless you submit a confirmation from Czech Police proving that the card was stolen. Then the fee is CZK 50). If you are applying for a contact chip card, its production process takes four weeks and the fee is CZK 300.
- Provided that you have stated your mobile phone number or email address in the application, we will inform you as soon as the card is ready to be collected at a contact point for card issue.
- When collecting your card, take with you the copy of Card issue application and come to the contact point stated at the application, or wait for the card to be delivered by post.
- If you had Public Transport vouchers downloaded on your lost card, contact point staff can download duplicates on your new card.
