If you want to cancel the card, you have to complete an Opencard cancellation form (PDF) and attach the card. The date of the cancellation form submission is also the date of the termination of contractual relations between the card holder and card issuer. The form is available at the issuer’s contact point.

2. Submit the form at an Opencard contact point
Submit the completed form at the Opencard contact point that is marked with the Opencard logo. On submission of the completed form, also present the card that you want to cancel, and a valid ID card. Foreigners must present either a valid passport or valid residency permit.
If you lose or damage the card, it is necessary to pay a handling charge of 200 CZK.
