There are two ways you can complete the form:
- Print the Notification form, complete it by hand and make 1 copy (or you can print 2 copies and complete both of them by hand); attach a colour passport size photograph, and take both copies to an Opencard contact point.
- Complete the Notification form on the computer (application requirements and instructions), print 2 copies, attach a colour passport size photograph, and take both copies to an Opencard contact point. The advantage of this type of form is that you can save it on your computer.
