Making a Complaint Due to Incorrect Printed Information

Before you send the form, it is necessary to complete all the fields that are compulsory, and marked with a star. In order to make communication easier, also complete the fields for mobile phone and e-mail address.

  • If the postal address agrees with the address stated in the original application, the card will be sent to the address stated in the form.
  • In case some of the stated data doesn’t agree with the data stated on the original application, the new card will not be issued without resolving the situation with the applicant. If the original card has the correct printed data, the new card will not be issued.

The original card will be blocked, and the holder is obliged to either hand it in Adria Palace, or to send it to the address:

By making a claim due to incorrect print, I also wish to apply for the issue of the new card. The original Application form will be used; its number is stated in this form.
